Founder and Director

Claire Plant
Claire has over 22 years of nursing experience, including specialist district nursing and more than five years as a Clinical Lead Nurse, She brings extensive clinical expertise, leadership, and a deep commitment to high‑quality care. As a Queen’s Nurse, she is dedicated to advancing community health and supporting people with complex needs. Her career spans hospitals, care homes, and NHS community services, where she has trained and mentored staff, led audits and after‑action reviews, and developed several new ways of working to ensure services are safe and effective. Alongside her healthcare background, she also brings over a decade of business and customer‑service experience in the retail sector.
Phebe Baker
Support Worker/Team leader
Focused, approachable, and driven by results, our support worker Phebe is all about building strong relationships. They help connect people to the right solutions—with clarity and care.
